The Role
Make a meaningful impact every day by supporting people living with dementia as they transition from hospital to home or into residential care. As a Dementia Support Coach, you’ll provide practical, relationship-based support, modelling and coaching for clients, carers and care staff across a variety of settings. This position offers professional growth and the opportunity to contribute to an innovative new project.
This role is offered as a Fixed Term Contract until February 2028 and is based out of our Cardiff office in New South Wales.

Responsibilities
- Provide on-the-ground support and implement relationship-based strategies for clients, carers and care staff
- Model and coach best-practice approaches to help carers and aged care staff understand and support people living with dementia
- Implement recommendation plans with guidance from the Team Leader, Dementia Consultant and Senior Consultant
- Facilitate and support the trial and implementation of brokerage items for clients and carers
- Collaborate with peers to build capability across aged care teams and support quality, person-centred care
- Strong organisational and time-management skills in a busy work environment
- Ability to work independently and collaboratively
- A commitment to excellent customer service and constructive conflict resolution
- A current driver’s licence and safe driving record
- Willingness to travel intrastate and interstate for up to five days at a time
- Flexibility to work varying hours based on client and carer needs
- Qualifications in a relevant field: Allied Health Assistant, Diversional Therapy, EN, Aged Care, Cert III/IV in Individual Support or Leisure and Health (highly desirable)
