The role
Community Connector working directly with individuals with learning disabilities, neurodivergent conditions, mental health conditions, or other disabilities. Act as a bridge between people, their families or carers, and the rich network of community resources available across Carmarthenshire. Help reduce social isolation, foster a sense of belonging, and support people to build confidence, independence, and wellbeing through meaningful community connections.
