Aspire Mentoring Foundation supports individuals with intellectual and developmental disabilities. Guided by our mission, "Doing the right thing for the right reason," we are a trusted resource for families in the Somerset community and surrounding areas.
In this rewarding role, you will oversee vital waiver services (Respite, CLS, Personal Assistance, Community Access, and Family Home Provider services) to ensure the health, safety, and ultimate quality of life for the people we support.
Key Responsibilities
- Program & Field Management: Conduct regular home visits, manage scheduling and supervision, ensure site/home compliance, and help organize meaningful community activities.
- Compliance & Care: Maintain accurate participant health records, attend Service Plan of Care meetings to establish quality goals, and ensure strict waiver service compliance.
- Team Leadership: Conduct interviews, participate in the hiring process, provide ongoing supervision to providers, and ensure all paperwork is completed accurately and on time.
- Matching & Support: Create positive, meaningful matches between participants and service providers. Provide temporary direct support if needed while actively hiring a permanent match.
- Culture: Bring enthusiasm to the role, foster independence, and have fun with our participants!
Position Requirements
- Minimum of one (1) year of direct experience working with Michelle P. and SCL (Supports for Community Living) Waivers. (Strictly required for consideration).
- High School Diploma or equivalent; must be 18 years or older.
- Valid driver’s license and current auto insurance.
- Excellent communication and interpersonal skills.
- Solid computer skills for digital record-keeping and compliance documentation.
