Summary

Reach Out provides companionship and practical support to older people who are medically vulnerable, home from hospital or at risk of going into hospital.
Responsibilities
Visit clients in their own homes, offer companionship, cup of tea and a good natter, or a short walk in the fresh air. The visit can make a difference to someone\’s day who is feeling lonely and isolated.
