Job Summary: The Special Needs Coordinator (SNC) Registered Nurse manages and oversees the Exceptional Family Member Program–Medical (EFMP-M) at the installation, ensuring timely identification, enrollment, data accuracy, and clinical oversight for military families with special medical and educational needs.

Required Qualifications

  • Education: Associate in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
  • License: Maintain an active, valid, current, and unrestricted RN license in any U.S. state/jurisdiction.
  • Certifications: Basic Life Support (BLS).
  • Experience: Minimum 3 years of full-time nursing and care coordination experience for ADN holders; 2 years for BSN holders.

Key Responsibilities

  • Oversee and manage the installation EFMP-M IAW DoD and Military Component policy, and any subsequent implementing guidance.
  • Use clinical assessment skills to identify sponsors whose family members have special medical and educational needs in a timely manner.
  • Collaborate with multidisciplinary service providers to ensure effective outreach and identification of special needs families.
  • Ensure family members of military personnel are appropriately Q-coded and enrolled in EFMP-M.
  • Maintain accurate data entry in military medical special needs management systems.
  • Provide clinical training to installation personnel and medical staff; consult as needed to support EFMP-M implementation.
  • Integrally involve in the Family Member Relocation Clearance (FMRC) process, coordinating enrollment and travel screening for active-duty families.
  • Provide clinical oversight to maintain a Special Needs file for each sponsor and conduct annual reviews.
  • Collaborate with Integrated Delivery System (IDS) and interagency forums to refer families to community resources.
  • Support integration of EFMP-M, EFMP-FS, and EFMP-A services at the installation.
  • Participate in quarterly case reviews to assess newly identified families and allocate appropriate resources.
  • Maintain records and documentation in compliance with HIPAA and DoD policies.
  • Complete required electronic medical record training, orientation, and SNC training programs.

Work Environment & Schedule: On site, Monday – Friday 7:30 – 16:30.
