Responsibilities include performing a full range of pharmacist procedures such as:
- Assessing patientβs response to drug therapy and planning drug therapy based on physician established diagnoses.
- Ordering and evaluating laboratory tests necessary to evaluate drug therapy effects and outcomes.
- Initiating, modifying, or discontinuing medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma).
- Monitoring and managing pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
- Initiating or modifying drug therapy for minor acute conditions such as colds, rashes, and allergies.
- Administering prescription or non-prescription drugs according to established agreements or protocols.
- Assessing metabolic needs and ordering therapeutic enteral or parenteral nutrition products in the inpatient setting.
- Evaluating medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
- Consulting with other healthcare providers about patient treatment needs or options.
- Running workload and throughput reports in MHS Genesis and attending professional staff meetings.
- Conducting controlled substance audits in the pharmacy and outlying clinics.
- Conducting and coordinating clinical investigations and research approved by a local or regional Investigational Review Board (IRB).
- Delivering comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.
- Counseling patients regarding appropriate use of medications.
- Reviewing patient profiles at the time of dispensing to monitor medication therapy closely.
- Ensuring prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients.
- Intervening with providers for noted discrepancies.
- Compounding medications as required.
- Ensuring accuracy of all pharmaceutical products prepared or processed by technical staff.
- Working closely with clinical staff in developing patient wellness programs.
- Providing medication information and therapy guidelines to clinical staff as required.
- Providing clinical direction to pharmacy personnel.
- Supervising inventory control of all pharmaceuticals in the pharmacy.
- Maintaining appropriate records and security of all scheduled controlled substances according to applicable regulations.
- Consulting with other specialty practitioners to provide optimal patient pharmaceutical care.
- Ensuring that work areas are clean and safe.
- Documenting significant medication interactions and pharmacy interactions with prescribers.
- Conducting Adverse Drug Reaction (ADR) reporting in accordance with TJC standards.
- Facilitating medical staff drug utilization reviews (DURs) as deemed necessary.
Work Environment: Work schedule days and times are Monday to Friday 07:00β16:00, 07:30β16:30, 08:00β17:00, 09:00β18:00, 13:00β22:00. This position does not require on-call or standby hours.
