Clinical Pharmacist - £54,600 A Year

Overview In this role you will be a key member of the general practice team, providing medicines expertise to improve patient outcomes and practice efficiency. You will act as the primary medicines contact across multiple sites within the Dean Medical Practice network, collaborating with GPs and other professionals. You will lead medicines optimisation, conduct reviews, and support safe, cost-effective prescribing while contributing to service-wide audits and improvements. Pay / Benefits stability and support of a larger organisation career development opportunities collaborative and community-focused culture focus on patient outcomes and professional development Responsibilities Ensure safe and effective medication management in collaboration with GPs and managers Lead medication reviews and structured medicines reviews Implement and reinforce a robust repeat prescribing system Design and implement safe prescribing for vulnerable groups and those with dependencies Promote cost-effective prescribing and address safety alerts Reconcile medicines post-discharge and for new registrations Review clinical reporting in Systm One and support KPI targets Create and run clinical reports and audits to improve practice Implement PCN-wide medication management policies Liaise with Pharmacy teams in PCN and ICB Mentor and develop pharmacy team members Support hypertension initiatives with digital pods and risk-stratified management Conduct non-medical/independent prescribing within scope Manage workload in a high-accuracy, high-pressured environment Comply with DBS checks and professional development requirements Key requirements Undergraduate degree in pharmacy, MPharm or equivalent Current General Pharmaceutical Council registration Complete or be prepared to undertake Primary Care Pharmacy Education Pathway (PCPEP) Essential: experience in medicines optimisation and prescribing in primary care (min 2 years preferred) Ability to work in a multidisciplinary team and autonomously DBS enhanced check satisfactory Willingness to pursue independent prescribing (desirable) Clear and polite telephone manner Polite and confident Flexible and cooperative Willingness to use Systm One (experience preferred) Office and Outlook proficiency (desirable) Strong IT skills
