We have an exciting opportunity for a Community Therapy Support Worker to join our Therapy Service in the Swadlincote Integrated Community Team across our core and rapid services.
Responsibilities:
- Deliver day‑to‑day therapy in the community, working alongside Physiotherapists, Occupational Therapists and Advanced Therapy Support Workers in patients’ own homes and care homes.
- Implement rehabilitation plans established by the team to achieve patients’ goals.
- Travel to visits and use a laptop to record notes in SystmOne.
- Manage caseloads and collaborate effectively with team members.
- Operate within our service hours: 08:00 am to 18:30 pm, seven days a week.
- Ensure behaviour and values reflect the DCHS Way.
Qualifications & Experience:
- Relevant experience in community or allied health support roles.
- High personal credibility and the ability to build effective working relationships across multiple settings.
- Strong teamwork, communication and organisational skills.
