We are looking to recruit a full‑time registered nurse to work within the Chesterfield Community nursing service, specifically in the community support beds at the Staveley Centre, supporting patients to return to their own homes. Our teams are friendly, supportive, and work closely together to meet the needs of the housebound population within Chesterfield.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations. Our purpose in Derbyshire is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

Responsibilities

  • The successful applicant will be responsible for providing evidence‑based care to support patients in their own homes and in the Staveley Centre.
  • The centre is a unit of community support beds for patients discharged from hospital with the aim of returning to their homes.
  • They will assess, plan, implement and evaluate individualised patient care, promoting self‑care wherever possible in an efficient and clinically effective way, while maintaining privacy and dignity.
  • They will provide advice, guidance and support to junior staff in the delivery of high‑quality care and actively support the District Nurse/Senior Community Nurse in the management of the health care environment.
  • They will work alongside members of the multidisciplinary team including therapy teams, general practice and specialist services.
