We are looking to recruit a NMC registered nurse to work within the Rapid Response team in High Peak and Derbyshire Dales.

We are a professional, friendly, and enthusiastic team who work closely with our integrated community colleagues to provide a holistic integrated service that enables and educates our patients to remain independent in their own homes, whenever possible, to prevent hospital admission and support a safe discharge home.

  • Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
  • Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient’s privacy and dignity.
  • Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.
  • Implement / refer on to the relevant services as appropriate.
  • Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
  • Responding to priority nursing visits.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire. We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
