We have an exciting opportunity for a Community Therapy Support Worker to join our Therapy Service in Chesterfield and North East Derbyshire.
Responsibilities
- Day‑to‑day delivery of therapy predominately in the Community Support beds at Staveley Centre, working alongside Physiotherapists, Occupational Therapists and Advanced Therapy Support Workers.
- Delivering rehabilitation plans established by the team to work on achieving the set goals and involving in the discharge planning process.
- Operating a laptop to write notes onto SystmOne, travelling to visits, and exhibiting excellent team working and communication to assist in caseload management.
- Maintaining service hours between 08:00am and 18.30pm seven days a week.
Qualifications
- Necessary experience of working in a therapy or rehabilitation team.
- High levels of personal credibility and the ability to instantly form effective working relationships in a range of work settings.
- Strong team working and communication skills.
