We are seeking an enthusiastic and motivated Band 3 Healthcare Assistant to join our Community First (D2A) team in Derby City.
Qualifications
- Previous experience of working in health and social care is essential.
- A good standard of English and Maths (GCSE or equivalent) and NVQ Level 3 (or equivalent) is required.
- Excellent communication skills.
- Access to and ability to use transport within the Derby and Derbyshire locality is essential.
Responsibilities
- Ensure your behaviour and values reflect those set out in the DCHS Way.
- Work as part of a wider integrated team providing holistic care to patients in their own homes and care home settings.
- Carry out patient care including wound care, catheter care and administration of injections.
- Visit patients within their own homes as part of the Community First D2A team.
- Support patients at Perth House, stepping down from hospital or stepping up from the community to prevent unnecessary admission.
- Follow prescribed care plans, implement and evaluate individualised patient care efficiently and clinically, maintaining patient privacy and dignity.
- Provide the delivery of high quality care and actively support the D2A nursing team in managing the healthcare environment.
- Implement relevant services in accordance with organisational protocols.
- Work in close collaboration with the DAU, UCR, and Integrated Community Teams.
