Note: This role is for trained ACPs ONLY. The role supports a core team of ACPs across all units to provide a model of seven-day service cover, ultimately linking trainees into a core clinical lead support resource. The GM will be responsible for ensuring that the direction of the service continues.

As an ACP you will also:

• Provide clinical leadership, including direct clinical care, and advice on complex issues in the UTCs.
• Establish a cohesive team approach for ACP workforce development.
• Support the UTC service leads and managers with audit, innovation, and performance management.
• Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps.
• Assist in feedback for staff appraisals.

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to adults and children, presenting with undifferentiated conditions both minor injuries and illnesses within a UTC setting. You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of adults and children. You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire. We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations. It is an exciting time to join primary care and community services as the national Ageing Well programme gathers pace. DCHS is working closely with the Primary Care Networks to transform the care of housebound and vulnerable people across Derbyshire through our Team-Up approach. We’re seeking ACPs with a special interest in the management of the typical conditions seen within a UTC, to provide day-to-day clinical leadership support to the teams.
