Daytime Healthcare are recruiting for an experienced Continuing Healthcare (CHC) Nurse Assessor to join an NHS team on a hybrid contract based in Birmingham. This role is an agency position that is initially 6 months.
We are looking for someone who can support the Lead Complex Manager and Commissioners, ensuring that the CHC programmes are effectively planned, delivered and reviewed.
Key Responsibilities Include:
- Coordinating timely quality assurance reviews for patients in specialist placements.
- Facilitating and contributing to multi-disciplinary assessments and patient reviews.
- Liaising with Local Authorities, other ICBs, and internal teams to ensure integrated service delivery.
- Promoting and implementing Personal Health Budgets in collaboration with families and internal teams.
- Supporting the Senior Clinical/Professional Lead with team business planning and performance reporting.
Requirements:
- Must hold a clinical or professional qualification (e.g., NMC or HCPC registered).
- Proven experience within CHC or a similar complex care environment.
- Strong understanding of safeguarding, legal processes, and multidisciplinary working.
Hybrid Working:
Expectation is that they will undertake face to face reviews of children in Birmingham and Solihull area. Otherwise home based and coming into the office for team meetings and training as needed.
