Day Webster are currently recruiting a Band 6 CPN to work with the community pathway team in Folkestone.
Job description:
- The assessment of both urgent and routine mental health presentations.
- Risk formulation, dialog care planning.
- Offering therapeutic intervention.
- System liaison (GP, social care, third sector partners).
- Clinic working, monitoring a person’s physical health and administering treatment for their long term mental health conditions.
- Care planning, risk formulation, safety planning and patient outcome measures.

- Physical health, depot and clozapine competencies.
