Day Webster are currently recruiting a Band 6 CPN to work with the community pathway team in Folkestone.

Job description:

  • The assessment of both urgent and routine mental health presentations.
  • Risk formulation, dialog care planning.
  • Offering therapeutic intervention.
  • System liaison (GP, social care, third sector partners).
  • Clinic working, monitoring a person’s physical health and administering treatment for their long term mental health conditions.
  • Care planning, risk formulation, safety planning and patient outcome measures.

Essential:

  • Physical health, depot and clozapine competencies.
