To deliver the Day One Casework bedside model to patients and their families impacted by serious or multiple injuries, providing support on wards, on-site clinics and other settings such as rehabilitation centres. To become embedded within the Trust, working with clinical teams focused on major trauma pathways and facilitating outpatient and in-person legal clinics. To engage those affected by serious injuries, take referrals and carry out initial assessments, providing signposting and connections to services immediately after major trauma and facilitating timely access to legal assistance for rehabilitation. To build relationships with key stakeholders, promote the Day One model and address the needs of patients and families affected by serious injury. To collaborate with the wider Day One Service team, managing records and data in line with regulatory requirements.
Key Responsibilities
- Work closely with NHS clinical staff and community and voluntary sector organisations to address the needs of patients and their loved ones affected by serious injury.
- Implement a case-management approach to assessing and identifying needs, connecting support, making referrals and providing direct assistance.
- Provide consistent assessment of all patients and families, offering early specialist legal advice to support rehabilitation and NHS cost recovery.
- Maintain detailed case records, including accurate activity logs, using Day One’s CRM system in line with policies and regulatory requirements.
- Contribute to report writing through case studies and narratives to support data collection.
- Collect and analyse patient and family feedback to improve Day One services.
- Deliver awareness-raising presentations to clinical colleagues and departments.
- Organise and facilitate outpatient and in-person legal clinic drop-in sessions.
- Build awareness of the role and charity’s purpose across the major trauma network.
- Collaborate with local authority, statutory and voluntary organisations to provide patient and family advocacy.
- Support the discharge process and establish ongoing support mechanisms outside hospital settings.
- Identify, recruit and supervise Day One volunteers where appropriate.
- Collaborate with the Peer Support Service to promote awareness and uptake.
- Work with the wider team on performance, quality, safety and effectiveness, ensuring safeguarding policies are followed.
- Support national and regional fundraising and communications initiatives.
- Travel across the region and occasionally to other sites and national meetings.
- Engage in continuous professional development, including mandatory Day One and NHS Trust training.
- Participate in external clinical supervision and monthly reflective practice sessions.
