Job Description
Summary
The Home Health Physical Therapist Assistant (PTA) performs treatment, provides exercise instruction, and patient education aimed at improving the patient\’s well-being while following the established plan of care.
Responsibilities
- Under supervision of a Physical Therapist, follow all state laws governing physical therapy in home care, adhere to the treatment plan set by the Physical Therapist, and perform only procedures qualified and trained for.
- Follow the plan of care, instruct and assist patients within the scope of practice and state laws.
- Observe, record, and report patient responses and condition changes to the supervising PT, nurse, and/or physician. Coordinate care with healthcare team members as appropriate.
- Instruct patients, families, caregivers, and other healthcare team members in therapy within the scope of the physical therapy assistant.
- Visit patients within the service area and nearby agencies as needed.
- Complete and submit progress notes for each visit according to policy.
- Participate in staff meetings and committees as requested.
- Adhere to LHC Therapy Standards of Practice.
- Participate in weekend rotations as required.
- Perform all other duties as assigned.
Education and Experience
License Requirements
- Current license, registration, or certification to assist in physical therapy practice in Florida.
- Current CPR certification.
- Valid driver\’s license, vehicle insurance, and access to a dependable vehicle.
