Make a Difference in Patients' Lives: UVM Home Health & Hospice is seeking a compassionate and dedicated LPN to join our growing Adult Home Health team! In this role, you'll play a vital part in helping patients receive high-quality care in the comfort of their own homes.
What you'll do:
- Provide skilled nursing care under the supervision of an RN, following established plans of care.
- Assess patients' physical, emotional, and social needs.
- Administer medications, wound care, and other treatments.
- Educate patients and families on healthcare needs and self-management strategies.
- Collaborate with other healthcare professionals to ensure comprehensive care.
What you bring:
- Active LPN license in Vermont.
- Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience.
- Strong assessment, communication, and interpersonal skills.
- Compassionate and patient-centered approach.
- Excellent teamwork and collaboration abilities.
