ABOUT THE JOB

As a Nutricia Homeward Nurse, you’ll provide care for patients who are being tube-fed in their own homes. You’ll build strong relationships with carers and healthcare professionals, including dietitians, GPs, and local nurses. This field-based position supports patients within Aneurin Bevan UHB and the surrounding area of South Wales.
To be considered for this role, you’ll need to be a Registered Nurse (RGN, RSCN or RNLD), with at least 12 months’ experience and living in South Wales or the surrounding area. You’ll also need a full UK Driving License; we’ll provide a company car, fuel card, and a lunch allowance when you’re in the field.
Role Focus:
- To develop and deliver comprehensive training to patients, carers, and HCPs to support safe discharges from the hospital, as well as ongoing training for quality care at home.
- Complete regular patient reviews and be on hand to support with questions, issues, and troubleshooting requests.
- To assist in preventing unnecessary hospital stays, including tube changes within the Community.
