Role Purpose As a Nutricia Homeward Nurse, you\’ll provide training and care for patients who are being tube-fed in their own homes. You\’ll build strong relationships with carers & healthcare professionals, including dietitians, GPs, and local nurses.
This role involves working with Adult & Paediatric patients in various areas of Southampton. You will need to be living in or able to relocate to the area.
Key Activities To deliver comprehensive training to patients, carers, and HCPs to support safe discharges from the hospital, as well as ongoing training for quality care at home.
Complete regular patient reviews and be on hand to support with questions, issues and troubleshooting requests.
To assist in preventing unnecessary hospital stays, including tube changes within the community.
About You You\’ll play a pivotal role in nurturing patient & HCP relationships. In return, we offer exciting projects and professional development opportunities for growth. Including supporting your Nursing & Midwifery Council re-validation and RCN membership.
Your ability to work independently, whilst effectively managing your diary.
Your clear understanding of the UK Healthcare Market, including the NHS.
How you\’ve used your excellent communication skills with internal and external stakeholders, including patients and HCPs.
Requirements You\’ll need to be a Registered Nurse (RGN, RSCN or RNLD), with at least 12 months\’ experience.
Be living in the area.
Full UK Driving License.
