About The Job
As a Nutricia Homeward Nurse, you’ll provide care for patients who are being tube-fed in their own homes. You’ll build strong relationships with carers & healthcare professionals, including dietitians, GPs, and local nurses. This role involves working with Adult patients within Sunderland and the Tyne and Wear surrounding area.
To be considered for this role, you’ll need to be a Registered Nurse (RGN, RSCN or RNLD), with at least 12 months’ experience and living within / close to the Sunderland/Tyne and Wear area of the account would be ideal. You’ll also need a full UK Driving License; we’ll provide a company car, fuel card and a lunch allowance when you’re in the field.
Key Activities
- To develop and deliver comprehensive training to patients, carers, and HCPs to support safe discharges from the hospital, as well as ongoing training for quality care at home.
- Complete regular patient reviews and be on hand to support with questions, issues and troubleshooting requests.
- To assist in preventing unnecessary hospital stays, including tube changes within the Community.
- You’ll play a pivotal role in nurturing patient & HCP relationships.
