We are looking for an enthusiastic, committed and experienced practice clinical pharmacist to join our clinical team and work with us in a friendly, well-established practice. The practice is flexible with working hours and both full time and part time applicants are welcome to apply.

The postholder will be a registered pharmacist with significant clinical and quality improvement experience and ideally an independent prescriber. The practice is looking for an individual who is committed to delivering a high standard of care to patients and is passionate about clinical pharmacy. The candidate must be able to work effectively as part of a diverse practice team. Although prior experience in General Practice is highly desirable, pharmacists from other sectors will be considered for the position.

Previous NHS general practice clinical pharmacy experience is highly desirable. Good working knowledge and proficiency with all Microsoft Office applications essential. EMIS Web knowledge desirable.

Hours

The successful applicant will need to work between 20 to 38 hours per week, Monday to Friday.

Conducts surgery and telephone consultations with patients with minor illnesses

Patient-facing medication reviews of patients with polypharmacy especially for older people and people with multiple co-morbidities.

Provide advice and support for patients with self-limiting conditions

Reconcile medicines following hospital or clinic discharge

Review medications for newly registering patients

Practice as an independent prescriber working within scope of practice

We are a well-established, friendly NHS GP Practice based in Dalston, Hackney, East London.

The practice\’s aim is to provide our patients with an excellent standard of patient-centred care, with clinicians and patients jointly involved in all aspects of planning and delivering care to meet patients\’ health and well-being needs.

You will work with colleagues including a team of GPs, practice nurses, administrative staff and a wide variety of other health professionals (physiotherapists, dieticians, diabetes specialist nurses, paramedics, physician associates, social prescribers and more). Job responsibilities

The postholder will work as part of a multi-disciplinary team to provide expertise in clinical medicines management, patient-facing structured medication reviews, manage long term conditions, manage medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, and acute prescription requests, while addressing the health and social care needs of the practice\’s patients.

Mainduties:

Provide advice and support for patients with self-limiting conditions

Reconcile medicines following patients discharge from hospital or clinic appointments

Review medications for newly registering patients

Implement and embed a safe, effective and evidence based repeat prescribing system

Practice as an independent prescriber working within scope of practice

Work as part of a multi-disciplinary team to manage repeat prescription authorisations and reauthorisation, acute prescription requests and systems for safer prescribing

Provide leadership to the practice on quality improvement and clinical audit as well as managing some aspects of the Quality and Outcomes Framework, and assisting with QOF and local performance targets

  • Occupational health clearance
  • Evidence of continuing professional development (CPD) commensurate with positions held

Qualifications

  • Degree in pharmacy, with postgraduate diploma
  • Registered pharmacist on the General Pharmaceutical Council register
  • Qualified independent prescriber

Experience

  • Minimum of 2 years post qualification experience
  • Experience and understanding of the range of primary care clinical work, including long term conditions
  • Experience of working autonomously
  • Comprehensive therapeutic and clinical knowledge and an understanding of evidence based healthcare
  • Experience of working in general practice as a clinical pharmacist
  • Experience of prescribing (independent prescriber)
  • Experience of working in a supervisory or mentoring role
