Weekday shifts (6am to 9am OR 5pm to 11pm) paid at £12.71 per hour. Weekend shifts (6am to 2pm AND 2pm to 10:30pm on a rota) paid at £15.00 per hour - you MUST be able to work every "other" Saturday and Sunday. Optional extra days and hours are also available during the week as a Home Care Worker.

Job Description
We are seeking a dedicated and organized Out of Hours (OOH) Care Coordinator to join our team in Hartlepool. The OOH Care Coordinator will be responsible for managing and coordinating care services during weekends, ensuring that our clients receive continuous and reliable support.
Key Responsibilities
- Coordinate and manage care services during weekends.
- Respond to emergency calls and provide immediate assistance as needed.
- Schedule and allocate care staff to meet client needs.
- Maintain accurate records of all care activities and communications.
- Liaise with clients, families, and healthcare professionals to ensure seamless care delivery.
- Provide support and guidance to care staff during out-of-hours shifts.
Requirements
- Previous experience in a care coordination or similar role.
- Excellent organizational and communication skills.
- Ability to work independently and make decisions under pressure.
