The Care Coordinator role at Daily Living Care LTD is a full-time, on-site position based in Birmingham. You will assess client needs, develop and update care plans, and coordinate home visits with support staff to ensure care is delivered safely and in line with each client’s preferences.
You will maintain case records, monitor service quality, respond to client queries, and work with the wider team to improve processes while ensuring compliance with care regulations and safeguarding
