Due to continued growth, we are seeking an experienced Care Coordinator to join our team and play a key role in ensuring safe, effective, and person-centred care delivery. To be considered for this role, you must have:

  • A minimum of 2 years' experience as a Care Coordinator within domiciliary care and/or complex care.
  • Experience managing care rotas, care packages, and staff deployment.
  • A full UK driving licence and access to your own vehicle.

Applications from Care Assistants or Support Workers without Care Coordinator experience will not be considered.

The Role

As Care Coordinator, you will be responsible for managing care delivery across a portfolio of service users, ensuring care visits are staffed appropriately and delivered to the highest standards. You will work closely with service users, families, care staff, healthcare professionals, and management to maintain excellent outcomes and service quality.

Key Responsibilities

  • Coordinate and manage care schedules and staff rotas.
  • Match care staff to service users based on skills, experience, and compatibility.
  • Conduct care reviews, spot checks, risk assessments, and service user visits.
  • Support the onboarding and supervision of care staff.
  • Ensure continuity of care and respond to changes in service user needs.
  • Assist with emergency staffing cover and last-minute schedule changes.
  • Maintain accurate records and ensure compliance with CQC regulations.
  • Work collaboratively with the management team to support service growth.

About You

You will be an organised and proactive individual who thrives in a fast-paced care environment. You will have:

  • Previous experience as a Care Coordinator within domiciliary or complex care.
  • Strong knowledge of CQC regulations and care standards.
  • Excellent communication and relationship-building skills.
  • Experience using electronic rostering and care management systems.
  • The ability to prioritise workloads and solve problems under pressure.
  • Confidence in conducting service user reviews, staff supervision, and quality monitoring.
