Bydd deiliad y swydd yn gweithio fel rhan o dîm o weithwyr proffesiynol. Yn yr amgylchedd newidiol hwn, bydd deiliad y swydd yn cynorthwyo’r Adran i gwrdd â’i chyfrifoldeb i weithredu Deddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) (2014) ac i weithio’n agosach â’r maes Iechyd a gyda phartneriaid eraill i sicrhau gwelliannau ledled y sector gofal cymdeithasol. Byddant yn rheoli llwyth achosion, yn cynnwys gwaith asesu a darparu ymyraethau gofal cymdeithasol, ac yn uwchgyfeirio achosion mwy cymhleth sy’n gofyn am gyfraniad proffesiynol ar lefel uwch gan Weithiwr Cymdeithasol / Therapydd Galwedigaethol.
Byddant yn hyrwyddo annibyniaeth, llesiant a diogelwch Oedolion, Plant a Gofalwyr ar Ynys Môn sydd gan anghenion gofal a chymorth. Bydd deiliad y swydd yn gwneud hyn drwy asesu anghenion Oedolion, Plant a Gofalwyr gan ddefnyddio gweithdrefnau’r Adran. Lle bo angen byddant yn asesu anghenion gofalwr/gofalwyr yr unigolyn ar wahân. Byddant yn datblygu a dylunio cynlluniau gofal a chymorth a chynlluniau priodol eraill e.e. cynlluniau Addysg.
Byddant yn argymell unrhyw gymorth allanol oddi mewn i gyllidebau, gan sicrhau bod y defnydd gorau’n cael ei wneud o’r adnoddau sydd ar gael a bod y gymeradwyaeth angenrheidiol yn cael ei rhoi lle bo angen e.e. Ceisiadau i’r Panel Adnoddau. Byddant yn cynllunio mewn modd creadigol gydag unigolion, gwneud y mwyaf o’r asedau sydd ar gael, lleihau’r risgiau a lleihau’r ddibyniaeth ar ddarpariaeth gwasanaeth tymor hir. Bydd hyn yn cynnwys rhoi gwybodaeth a chyngor, cefnogi pobl i ddefnyddio adnoddau cymunedol cyffredinol, darparu offer ac addasiadau cymunedol, a sicrhau mynediad at wasanaethau gofal a chymorth statudol yn ôl yr angen.
