An opportunity has arisen for an enthusiastic individual who is seeking a career in our Carers and Community Support Team as a Team Around the Person Co-ordinator. This role is funded by the Regional Integration Fund and is Fixed Term until 31st March 2027.
The role of the Team Around the Person Co-ordinator aims:
- To develop systems within communities for the co-ordination of preventative and early intervention support for vulnerable adults.
- To work with community organisations to develop their networks and draw together organisations that can offer support for community members.
On a day-to-day basis you will:
- Work alongside partner organisations in identifying need and innovative solutions to meet need within communities.
- Develop processes and protocols that will enable the co-ordination of services.
- Co-ordinate the work of a multi-agency Team Around the Person.
We are looking to recruit an individual who:
- Has the ability to think creatively and innovatively and translate ideas into actions effectively.
- Is able to develop productive professional relationships with key stakeholders.
- Has experience of developing new systems of practice.
- Has experience of leading on Community Based projects.
In addition to the above, we require an individual with a degree of fluency in the Welsh language. A successful applicant without the required Welsh language skill will be fully supported to reach the desired standard within two years of appointment.
