The Community Resource team Home Care (CRT) are part of Cardiff Council’s Independent Living and Community Care Service (ILS) and work in partnership with vulnerable adults to support them to live independently at home and connected to their communities.
CRT is a joint service provided by Cardiff Council and Cardiff and Vale University health board that aims to support adults, through therapy and/or home care, to recover or maintain their ability to live independently at home. The service provides reablement care which aims to encourage and support people to learn or re‑learn skills necessary for daily living, following a period of illness or after a stay in hospital.
About the job
An exciting opportunity has arisen to join Cardiff’s CRT Home Care team.
You will work as part of a team of Schedulers to ensure effective deployment of Reablement Care Workers across Cardiff, utilising appropriate databases and electronic call monitoring systems, to enable safe and effective service delivery.
You will proactively work within the team. Effectively communicating and providing back-office support to Reablement Care Workers in the provision of an efficient and effective service.

What We Are Looking For From You
We are looking for a dynamic person who has experience of working in a Home care setting and who can effectively deploy Reablement Care Workers, to meet the assessed needs of individual service users, including covering for planned and unplanned absences in line with policies and procedures.
You will need the ability to utilise appropriate databases and electronic call monitoring systems, to enable safe and effective service delivery.
You will have excellent communication skills and a good knowledge of working within a multi-disciplinary team.
This role includes rota’d weekend working.
The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.
