SOCIAL WORK ASSISTANT in Cardiff

Adult Community Services is our largest service area, supporting adults aged 18 and over, including older people and those with physical disabilities. Our teams work closely with individuals, families and carers from their first contact with Adult Social Services, helping them to access the right support at the right time.

As a Social Work Assistant, you will play a key role in supporting Social Workers and the wider multi-disciplinary team to deliver high-quality services. Working alongside Social Workers, Occupational Therapists, health professionals, Day Centre staff and other partner agencies, you will assist with assessments, support planning, reviews and ongoing interventions, helping individuals to maintain their independence and wellbeing within their communities.

We work closely with a wide range of home care providers and residential care services across Cardiff to ensure people have access to the support that best meets their needs and helps them achieve their personal outcomes.

Within Adult Community Services, several teams work together to ensure individuals receive the right support, in the right place, at the right time:

• First Point of Contact and Adult Assessment Team
• Case Management Team
• Review Team
• Community Duty Team

These teams provide excellent opportunities for Social Work Assistants to develop their skills and experience while making a positive difference to the lives of adults and their families across Cardiff.

About the job

This post is for a Grade 6 Social Work Assistant within our Community Duty Team. The service area is made up of a diverse range of professionals, including newly qualified and experienced Social Workers, Social Work Assistants, Senior Social Workers, Team Managers and a Service Manager, all working together to provide high-quality support to adults across Cardiff.

As a Social Work Assistant, you will play an important role in supporting Social Workers to deliver person-centred services. You will work directly with individuals, their families and carers, gathering information, undertaking agreed tasks and interventions, supporting assessments and reviews, and helping people to access appropriate services and resources to meet their needs and achieve their personal outcomes.

This role offers an excellent opportunity to develop your knowledge, skills and experience within a professional, supportive and hard-working team. You will be involved in providing practical support to citizens, helping to respond to a wide range of enquiries and situations, some of which may be complex and require a sensitive and professional approach.

Working closely with colleagues across Adult Community Services and partner organisations, you will contribute to effective decision-making, maintain accurate records, and support the smooth operation of the Community Duty Team. You will also have opportunities to contribute to service improvements, share ideas, and support the ongoing development of high-quality services for the people of Cardiff.

What We Are Looking For From You

We are looking for a motivated and compassionate individual who is committed to supporting adults to achieve positive outcomes and maintain their independence. Experience of working in a health, social care or related setting is desirable, and you will have a genuine interest in supporting older people and adults with physical disabilities.

As the successful candidate, you will be able to work effectively both independently and as part of a team, managing your workload and prioritising tasks in a busy environment. You will have excellent communication and interpersonal skills, enabling you to build positive relationships with citizens, carers, colleagues and partner agencies.

You will be able to gather and record information accurately, maintain clear and concise case records, and contribute to assessments, reviews and support planning under the guidance of qualified Social Workers. Good organisational skills and confidence in using IT systems are essential.

You will be committed to working in a strengths-based, person-centred and outcome-focused way, supporting adults to identify and achieve what matters most to them. You will understand the importance of promoting dignity, choice, independence, equality and inclusion in all aspects of your work.

We are looking for someone who is keen to learn, develop their skills and contribute to the continuous improvement of services. You will be a reliable team player who demonstrates professionalism, enthusiasm and a commitment to delivering high-quality support.

A relevant qualification in Health and Social Care, or equivalent experience, would be advantageous. The ability to speak Welsh or other community languages is also welcomed.
