Key Responsibilities
- Provide a single point of access for all referrals into the Memory Service within Older Adults Mental Health.
- Ensure all referrals are screened in line with the All Wales Mental Health Triage Scale and categorised appropriately to manage risk.
- Liaise with multi-agencies to establish a comprehensive assessment of referral information, utilising analytic and professional judgement when reviewing individuals/care partners’ needs.
- Impart and discuss sensitive information with care partners if there is significant risk identified or lack of capacity.
- Collate and analyse statistical data from the current database to facilitate the triage of all referral information.
- Make autonomous clinical judgement to prioritise and allocate the referral to the appropriate professional within the service or suggest alternatives to the referrer.
- Review all referrals to identify any gaps in the referral process to strengthen existing joint partnership systems for improving early and appropriate referrals.
- Support the administration of collating statistics and the development of a dementia register in line with the All Wales Dementia Care Pathway of Standards.
- Work as part of a multidisciplinary team to support colleagues within the community, ensuring the delivery of a high-quality service for older adults with mental health needs.
- Collaborate with the local authority, third party sectors, and other disciplines within CTMUHB.
- Undertake supervision responsibilities for staff within the team.
