Join the innovative Intermediate Care (Hospital@Home) team at Cwm Taf Morgannwg University Health Board. This is a great opportunity to work in a dynamic, multi-disciplinary environment where your expertise will improve patient outcomes and support independence at home.

The successful candidate will provide professional and operational management to pharmacy technicians working within the Intermediate Care (Hospital @ Home) service. They will play a key role in the development, coordination and delivery of medicines management support by the Hospital@Home service to patients in the community and following their discharge from hospital. In addition to overseeing a team of pharmacy technicians, the post holder will assess patients’ ability to manage their medication and support them with tailored care plans. The senior technician will work alongside clinical pharmacists and other healthcare professionals within a newly established multi-disciplinary team. Coordinating with hospitals, GPs, community pharmacies and social care providers, the post holder will be integral to the delivery of safe and effective care. Working independently but within a supportive team, the role will utilise clinical skills and evidence-based approaches to improve patient outcomes.

Responsibilities

  • Provide professional and operational management to pharmacy technicians within the Hospital@Home service.
  • Develop, coordinate and deliver medicines management support to patients in the community and after hospital discharge.
  • Assess patients’ ability to manage their medications and implement tailored care plans.
  • Collaborate with clinical pharmacists and other healthcare professionals; coordinate with hospitals, GPs, community pharmacies and social care providers.
  • Work independently within a supportive multidisciplinary team to improve patient outcomes.

Qualifications

  • Qualified Pharmacy Technician.
  • Experience in community or hospital-based medicines management is desirable.
