Principal Pharmacist Primary Care (Cluster Development) Join to apply for the Principal Pharmacist Primary Care (Cluster Development) role at Cwm Taf Morgannwg University Health Board. The post holder will be responsible for managing, leading, developing and delivering primary care medicines management services for Cwm Taf Morgannwg University Health Board (CTMUHB); developing the medicines management service model and delivery plan for the transformation multidisciplinary teams (MDT) within primary care. The post holder will support the Chief Pharmacist to develop and implement the UHB medicines optimisation strategy and plan by ensuring our Cluster Pharmacy Teams are aware of, and aligned with the medicines management plans for safe, evidence based and cost-effective primary care prescribing. The post holder will be the medicines management lead on key strategic, operational, transformational programmes and initiatives within primary care requiring medicines management input.
