Cwm Taf Morgannwg University Health Board is seeking a Dementia Intensive Support Team member to provide care and support to older adults with dementia in their own homes, under multidisciplinary supervision.
You will help implement care plans, monitor symptoms, support independence, and work closely with families to enable recovery and reduce need for inpatient care across CTM regions (Bridgend, Merthyr Tydfil, Rhondda Cynon Taf).
