The post holder will work independently in a variety of Community and Primary Care settings across areas of highest deprivation in Cwm Taf Morgannwg University Health Board (CTMUHB), as part of the Wellness Improvement Service (WISE) Programme Team. They will roll out the Wellbeing Facilitator Support across the CTMUHB area, using a variety of methods including group education sessions and individual person support to activate individuals to improve their health and wellbeing.

Main duties of the job

The post holder will be expected to undertake appropriate training to use WISE associated software and also to develop excellent communication and behaviour change skills which will enable them to support and motivate patients to make positive lifestyle changes, identifying their own health associated risk factors and enabling them to reduce and manage their risk or long term health condition.

Welsh Skills

Desirable Welsh skills are desirable for this role. This does not mean the position is essential; while the candidate does not need to have skills in Welsh, we will consider it an advantage when short‑listing and selecting candidates. This isn’t "fluency", just speaking & listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.

Person Specification

  • Welsh language speaking/listening skills at Level 3 or above
  • Community working experience
  • Experience of working within a primary, secondary or community care setting or equivalent
  • Good understanding of local services and communities
  • Experience of working in or contributing to local community groups (paid or unpaid)
  • Experience of working with disadvantaged communities and social care
  • Motivational support
  • Motivational interviewing and brief intervention training
  • Experience of working with individuals and delivering motivational one-to-one support

Knowledge

Knowledge of primary and social care systems and how to assist people to implement change.

Qualifications

  • Level 3 Diploma in Clinical Healthcare Support, Fitness Instructor or equivalent qualification or experience
  • Completed venepuncture training or willingness to undertake
