Exciting opportunity to join our specialist stroke early supported discharge team covering Merthyr Cynon, Bridgend and surrounding areas. The role involves working with patients who have experienced a stroke and require further rehabilitation within their own homes to maximise independence. This will include completing assessments and interventions in the patient’s property and local community, with opportunities to access acute and rehabilitation sites.

Main duties:

  • Work alongside registered Occupational Therapists and manage a delegated caseload of patients.
  • Support the assessment and treatment of people following a stroke who may have a range of functional difficulties within the bounds of the technician role.
  • Patient management will include responsibility for the delivery of care, including liaison with patients, family, members of the MDT and external agencies.
  • Good time-management, prioritisation skills and flexibility to the service are required.
  • Effective communication both verbally and in writing is essential.
  • Predominantly work within the community and relevant hospital bases as required.
  • Be an integral part of the team and have a key role in the delivery of high quality interventions.
  • Support will be provided by the Senior clinicians and the Clinical Lead, with access to supervision and in-service training opportunities.

Requirements:

  • Achieve level 3/level 4 Agored qualification in Occupational Therapy Support.
  • Welsh speaking is desirable; Welsh and/or English speakers are equally welcome to apply.

Physical requirements:

  • Physically able to move and handle patients on own.
  • Able to carry and fit equipment in patients' home on own.

Circumstances:

  • Able to travel between sites in a timely manner.

Knowledge:

  • Understanding of stroke and its impact on patients.

Skills:

  • Verbal and written communication skills.

  • Welsh language speaking/listening skills at Level 3 or above.

Experience:

  • Competency based training in OT NHS or rehab experience.

Education:

  • OT OCN or NVQ level 3 or equivalent experience.
  • Agored completion.
