As part of a close MDT, you will work together with your patients to identify meaningful goals, provide assessments, identify appropriate interventions, and support their independence and discharge from hospital. The post holder will carry an independent caseload of patients and deliver occupational therapy interventions to a high standard, providing evidence‑based, high‑quality care. The successful candidate will work closely with the MDT, Principal OT, Discharge Liaison Team and Local Authorities to ensure effective cross‑boundary and agency working.

Main duties of the job

  • Provide specialist Occupational Therapy assessment, intervention, rehabilitation and discharge planning for patients to ensure a safe and timely discharge and to enable the person to remain at home.
  • Provide supervision and support to junior members of the team and student Occupational Therapists, and promote evidence‑based, quality intervention using research, audit and quality improvement projects.
  • Flexibility to service needs, excellent self‑management skills and efficient caseload administration ensuring intervention focused on preventing avoidable admissions, rehabilitation and efficient discharge planning.

Person Specification

Qualifications and/or Knowledge

  • Degree/Diploma in Occupational Therapy
  • Registered member of HCPC
  • Documented evidence of CPD
  • Post‑graduate training in motivational interviewing, goal planning
  • Post‑graduate training/accreditation in fieldwork education
  • Membership of professional body

Experience

  • Experience of autonomous caseload management, including assessment, planning, intervention and evaluation within relevant clinical areas.
  • Understanding the OT process with particular emphasis on physical conditions
  • Supervision of staff
  • Understanding of current best practice in OT with designated client groups
  • Knowledge of the principles of clinical governance
  • Knowledge of a range of policies/procedures (e.g., risk management, protection of vulnerable adults, DOLs)
  • Experience of audit
  • Experience of multidisciplinary working
  • Experience of working in the community
  • Experience of service delivery in a variety of different clinical physical health settings

Aptitude and Abilities

  • Ability to work single‑handedly with individuals or groups

  • Demonstrable clinical reasoning skills and effective clinical decision‑making following assessment of complex cases
  • Knowledge of a broad range of medical, surgical and orthopaedic conditions

  • Ability to undertake the physical activity required for handling of clients
  • Effective written and oral communication skills
