Are you an experienced Mental Health Nurse looking to utilise your clinical skills in a different and rewarding environment?

The Dementia Intensive Support Team (DIST) are seeking an ambitious and caring Band 6 Registered Mental Health Practitioners to join our existing team.

The DIST team are proactive and highly skilled at managing risk and supporting service users within CTM who may be experiencing or approaching crisis in their mental health. The service is passionate about providing alternative options to hospital admissions, so you can expect to be undertaking rewarding and highly intensive work.

We looking to recruit a compassionate, dedicated, and conscientious individual, who wants to make a positive difference to adults experiencing a dementia-related crisis.

Staff provide care, treatment, and assessment to patients in their own homes and other places of safety, getting you out and about seeing patients in a range of community settings.

The post holder will facilitate rapid assessment, treatment, crisis prevention and intervention along with preventing avaoidable hopsital admissions

Will be responsible for undertaking comprehensive clinical assessments. They will provide specialist clinical advice to other team members on the suitability of various assessents/treatment/support.

Will be clinically resposnible for spporting the delivery of a flexible, responsive local service within the community.

You will work collaboratively with collegues, key partners and those within the local community to develop services to meet the mental health needs of the community.

Assess risk and formulate safety plans accordingly.

Contribute to effective communication with all parts of the mental health service, in particular the Older Adults Community Mental Health Teams (OACMHT).

Establish rapport and maintain supportive professional relationships with clients and their carers, even when there are barriers to acceptance such as lack of insight or difficulties adjusting to complex changes; recognising the stress associated with caring, offering and promoting carer’s assessments as required.

Deputise for the team leader as and when required.
