Cwm Taf Morgannwg University Health Board are offering an exciting and challenging opportunity to work as Continuing Health Care / NHS Funded Nursing Care Team Leader.
We are looking for a professional who is reliable, flexible and self-motivated who can work as part of the Nursing Team across Cwm Taf Morgannwg UHB and has a working knowledge of both the Continuing Health Care Framework and NHS Funded Nursing Care where care is provided via the Independent Sector, residential settings and the community.
You will manage a Nurse Assessor Team and work with the Continuing Health Care Senior Nurse in developing, coordinating and sustaining a Continuing Health Care model and service to meet the needs of the population of Cwm Taf Morgannwg University Health Board area.
