Cwm Taf Morgannwg University Health Board in Bridgend is redesigning the Intermediate Care Service and recruiting an Advanced Clinical Practitioner to join the Hospital at Home team. The role will deliver early supported discharge and provide autonomous clinical assessment and management in patients' homes.
You will work with Consultants, Nurses, Physiotherapists, OTs and social care colleagues to support safe discharge, promote independence and ensure high-quality care seven days a week.
