Background
Improving physical health care for people through NHS Health Checks.
Purpose:
The primary goal is to screen patients and conduct Full NHS Health Checks and Mini Health Checks in community venues, reducing the incidence of cardiovascular disease (CVD) by referring to GPs and other services.
Eligibility:
Individuals aged 40-74 who are not on a pre-existing vascular disease register.
Preventative Focus:
It targets 6 major risk factors: high blood pressure, smoking, cholesterol, obesity, lack of physical activity, and alcohol consumption.
Dementia Awareness:
People aged 65-74 are provided with information on reducing the risk of dementia, which shares common risk factors with vascular disease.
Purpose of the role
The main purpose of the role is to reach those individuals in high-risk areas of CVD to conduct a thorough NHS Health Check.
- Commissioned through Public Health Wirral, Local Solutions aim to increase the uptake of NHS Health Checks out in the community for individuals that do not engage or find it difficult attending their GP practice.
- This role includes working closely with the Head of Social Care and the local community venues where it will be based.
- You would be required to undertake each aspect of the NHS health check with full training provided.
Role Specific Key Tasks
- The ability to plan, set goals and identify priorities including booking of venues
- The collection and updating of relevant data
- To use Health Diagnostics for data inputting
- When necessary to contact individuals who are eligible to attend for a Health check
- When required, to attend meetings with Local Solutions and/or Public Health - Wirral
- To identify potential opportunities for support and health promotion which may include onward referrals
- To carry out health checks consisting of blood pressure readings, a BMI test, HbA1c & Lipid testing, alcohol and smoking screening, lifestyle and family history questions
- To undertake training at Local Solutions
- To provide and collect qualitative information including case studies and service feedback
- Manage and prioritise a caseload when needed
- To attend team meetings, supervisions etc. as required
Data capture
- Use Health Diagnostics (training provided) to survey each individual for a health check for performance monitoring
- To collect 6-12 follow feedback from individuals after receiving a health check
- To provide a case study once per quarter
Professional development
- Complete all statutory and mandatory training
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety
- To attend all necessary meetings with Local Solutions which include Supervision, Team Meetings etc.
Miscellaneous
- Work as part of the team to seek feedback, continually improve the service and contribute to business planning
- Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner
- Venue location may vary from time to time, without changing the general character of the post or the level of responsibility
- A driving licence & access to a vehicle is essential
- Role is dependent on a DBS check
