We’re currently looking for an experienced Band 6 Continuing Healthcare (CHC) Assessor for an exciting 4-month contract.

The Role: You’ll play a vital role in delivering high-quality CHC assessments across the local population. This position requires a confident practitioner with a strong understanding of the CHC framework and the ability to carry out both Decision Support Tools (DSTs) and Reviews in line with NHS National Framework guidelines.

Location: Remote working with some community-based visits

Duration: Initial 4 months

Rate: £280 per day (via umbrella)

Start Date: ASAP

Key Responsibilities:

  • Complete CHC assessments, DSTs, and Reviews both remotely and in the community
  • Work within the NHS National Framework for CHC
  • Liaise with multidisciplinary teams and family members
  • Maintain accurate and timely documentation
  • Support the ICB/CSU team with case management when required

Requirements:

  • Registered Nurse, Social Worker or Allied Health Professional
  • Strong CHC experience, particularly in DSTs and Reviews
  • Familiar with the NHS National Framework for CHC (2022)
  • Ability to work autonomously and manage caseload effectively
  • Access to own vehicle (for community visits, if required)
