Working Pattern: 08:00 ā 20:00, 4 days one week and 3 days the following week, this includes alternative weekend work.

What Weāre Looking For:
Weāre seeking compassionate, dedicated Wellbeing Support Practitioners to join our team. If you have experience supporting adults with complex needs and want to make a real difference, we would love to hear from you.
You will be responsible for providing personācentred and comprehensive care to an individual with complex learning disabilities, including assistance with personal hygiene, administering medication, PEG feeding and ensuring they have a meaningful life within the community, engaging in all aspects of available opportunities, all delivered with respect and compassion. Our personācentred approach ensures that each individual receives the specific and tailored care and support they need to thrive as an active citizen.

Week 1: Monday, Tuesday, Saturday, Sunday
Your hours: 08:00 ā 20:00
Key Responsibilities:
- PersonāCentred Care: Develop personalised care plans, ensuring service users' voices are central to decisionāmaking.
- Promote Independence: Support individuals in building daily living skills, helping them live as independently as possible.
- Community Inclusion: Facilitate access to social and recreational activities, encouraging participation in community life.
- Emotional Support: Provide emotional and social support, promoting positive mental health and wellābeing.
- Advocacy: Ensure service users have a say in decisions affecting their lives, supporting informed choices.
- Health and Wellābeing: Administer medication, assist with healthcare appointments, and promote healthy lifestyles.
- Record Keeping: Maintain accurate documentation, ensuring compliance with care standards.
Qualifications and Experience:
- Experience in a care setting, ideally with adults with learning disabilities, autism, or mental health conditions.
- Strong communication and interpersonal skills.
- Compassionate, patient, and understanding of complex needs.
- Ability to work independently and as part of a team.
- Knowledge of safeguarding principles.
- Basic understanding of personal care routines and medication administration.
- A valid driver's license and access to a vehicle.
- NVQ Level 2 or 3 in Health and Social Care.
- First Aid certification.
- Experience planning recreational activities.
