This role plays a key part in shaping and strengthening a therapy‑led reablement service aligned with national best practice. You will support Reablement Managers to enhance the therapeutic offer, designing, implementing and embedding meaningful improvements that deliver real outcomes for individuals.
Responsibilities
- Support Reablement Managers to enhance the therapeutic offer, designing, implementing and embedding meaningful improvements.
- Lead change within a dynamic environment, driving service development and overseeing agreed improvement actions.
- Collect and evidence the impact of change to deliver real outcomes for individuals.
- Develop graded, therapy‑led programmes that promote independence, maximise individual strengths and improve quality of life, supporting prevention and long‑term wellbeing.
- Champion the service, building positive relationships with internal and external stakeholders.
- Inspire and guide managers and frontline staff to create lasting improvements.
Qualifications and Experience
- Proven leadership skills and experience in service improvement or change management.
- Commitment to person‑centred outcomes and continuous improvement within a reablement or related service.
- Strong communication and relationship‑building skills.
Required Certifications
Enhanced DBS check with an Adults Barred List check is required.
