Responsibilities
- Manage a complex, high-risk caseload.
- Assess needs and design person-centred care plans.
- Deliver empowering outcomes and champion a re-ablement approach.
- Ensure safety, dignity and independence for individuals.
- Use professional judgement and innovative, solution-focused thinking to shape services.
- Collaborate with health partners, community organisations and other agencies.
- Provide case supervision and professional guidance to colleagues.
Qualifications & Requirements
