Role Overview

This is a specialist role where you’ll manage a complex, high-risk caseload, applying your expertise to assess needs, design person-centred care plans, and deliver empowering outcomes. You’ll champion a reablement approach, helping individuals reach their full potential while ensuring safety, dignity, and independence. With significant autonomy, you’ll use professional judgement and innovative, solution-focused thinking to shape services and improve lives across Cumbria. Collaboration is key, you’ll work closely with health partners, community organisations, and other agencies, while also providing case supervision and professional guidance to colleagues.
