This is an exciting opportunity to join a dynamic, fast moving Rehabilitation ward based in North West Surrey, Woking Community Hospital. The ward provides twenty-four hour care to patients who require rehabilitation and support prior to getting back to their chosen place of residence. This role offers an opportunity to acquire and develop skills as a healthcare assistant. The ward works closely with the local Universities to support student nurses and will work with individuals to develop personal and professional growth. The job requires the individual to work days and nights on a rota basis.

Main duties of the job:

  • To support and assist the ward team members in the provision of a high standard of patient care, including the promotion of physical, mental and emotional wellbeing to the patient.
  • To be an effective member of the ward Multi Disciplinary Team carrying out assigned tasks involving direct and indirect care provision to patients under the supervision and guidance of senior staff members.
  • The HCA role will work across a twenty-four hour/seven day week period within the ward setting to deliver direct personal/nursing care to patients.
  • To undertake tasks on the ward of a non-clinical nature which contribute to the smooth running and general management of the ward.

Job responsibilities:

  • Communication, planning, and analytical skills: To develop a rapport with patients, which encourages their participation in a manner which promotes dignity, self-esteem and choice.
  • To employ the use of effective communication, verbal and non-verbal to communicate with patients who may have barriers to understanding or communication eg. sight, hearing, language or cognitive impairment.
  • To communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information or handing over responsibility for care.
  • To be aware of, and understand, the individual's physical, emotional, psychological, social, spiritual and cultural needs.
  • To demonstrate an awareness and understanding of patients' physical conditions.
  • To report back to the MDT and/or senior colleagues any changes or concerns in functional abilities, information gained and feedback from patients.
  • To maintain legible records of all patient interventions and management, in accordance with the standards set by the organisation.
  • Recognise and report any situations, behaviour or errors that could result in poor care outcomes.
  • Be aware of the procedures in place to protect vulnerable adults.
  • Support patients to improve and maintain their mental, physical, behavioural health and wellbeing.
  • To undertake direct patient care which includes, but is not restricted to: The undertaking of clinical observations eg Blood Pressure, Pulse, Respirations; The recording of the observations on a National Early Warning Score chart; Urine Testing; Blood Sugar monitoring; Taking of swabs and specimens; Stoma and Urinary Catheter management.

  • Demonstrate ongoing continuing professional development.
  • Attend statutory, mandatory, in-service training and any other training programmes identified as part of your personal development.
  • Maintain professional behaviour and attitude at all times, and demonstrate excellent customer care skills in line with the CSH Values.
  • Adhere to and be familiar with CSH policies e.g. Uniform, Zero Tolerance etc.
  • To be aware of and demonstrate adherence to, CSH policy on infection control and waste disposal.
  • To adhere to the principles of universal precautions in all aspects of patient care.
  • Complete required CSH competencies, as applicable to role.

Person Specification:

Experience:

  • Recent experience of delivering direct patient care.
  • Minimum 1 year of UK-based or equivalent healthcare sector.
  • Experience of working within a ward based environment.
  • Experience of working with older adults with mental health needs.
  • Experience of working alongside Physiotherapists and/or Occupational Therapists.

Qualifications:

  • English Language and Mathematics GCSE, or equivalent literacy and numeracy qualification.
  • Evidence of recent professional development relevant to the clinical area.
  • Care Certificate.
  • Health & Social Care NVQ Level 2.

Skills:

  • Good written and verbal communication skills.
  • To identify the needs which patients may have during their hospital stay including physical, psychological, cultural and social.
  • Flexibility and adaptability to respond to changing situations and timescales.
  • The ability to recognise when there are changes in the patients condition and seek advice from trained staff.
  • Ability to work independently and as an effective part of a team.
  • Good interpersonal skills.
  • Basic competency in keyboard skills.
  • To promote patients independence and continuity of care.
  • Advanced IT / computer skills including the use of Microsoft Word, Excel and Outlook.
  • Use of electronic patient records eg.EMIS.
