The Integrated Neighbourhood Teams are looking for a Band 5 Community Nurse to join our community nursing team across the neighbourhoods of Walton, Weybridge and Hersham, Chertsey and Ottershaw and Addlestone. The Community Team visit patients in their own home as well as care homes. We offer excellent opportunities for training and development of both your clinical and leadership skills. The team has experienced Senior District Nurses who will ensure that you have the necessary support for your personal development. The community nursing service works over a 7-day week 8.00am to 8.00pm offering a variable shift pattern and includes weekend and bank holiday working. This post requires an Adult Nursing qualification. Newly qualified nurses are encouraged to apply for this position. You will be supported with a comprehensive induction and training package as needed, to help you settle in, with the time and opportunity to get to know the people on your caseload, along with development opportunities.

Main duties of the job:

  • Participate in assessment and care planning of patient care in conjunction with the District Nurse.
  • Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan; escalate any deviation from the care plan or change in condition to the District Nurse / GP accordingly.
  • Communicate effectively with patients, carers and all members of the multidisciplinary team.
  • Prioritise daily work in line with clinical / patient need.
  • Maintain accurate and legible patient records in line with professional and organisational requirements, including contemporary paper records and electronic patient record system.
  • Implement and work to ensure evidence-based care / best practice.
  • Actively participate and contribute to team meetings and daily handover.
  • Initiate health promotion activities with individuals, patients and carers.
  • Take responsibility for a defined caseload in the absence of the District Nurse, under the direction of the Practice Teacher / Team Leader.
  • Be actively involved in the development and implementation of community nursing service developments and initiatives.
  • Participate in clinical audit and the implementation of changes to ways of working / clinical practice as a result of findings, recommendations and action plans.
  • Participate in risk assessment and promote the safety and well-being of patients, staff and any other persons at all times and assist in ensuring a safe work environment.
  • Ensure that all complaints, untoward incidents, accidents and areas of clinical risk are identified and reported to the District Nurse in accordance with the organisation policies.
  • Keep up to date with professional requirements, health care developments, clinical evidence, National Service Frameworks for practice, National and Local policy and guidelines.
  • Be responsible for completing all own mandatory training within the required timeframes.
  • Access and participate in regular clinical supervision and peer review.
  • Participate in own annual appraisal and undertake own continuing professional development in order to maintain and develop knowledge and skills.
  • Access regular 1-1 meetings with District Nurse.
  • Undertake mentorship training, facilitate the teaching and learning experiences for all students and staff.
  • Be accountable for own professional actions in line with the NMC code and organisational policy and guidelines.
  • Monitor and order stock levels and equipment in line with budget and lean principles.
  • Be responsible for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained.

Person Specification:

  • Other: Current driving licence and access to a car during the working day. Ability to travel across bases and patients' homes. Flexible and adaptable.
  • Experience: Knowledge of recent developments within health and social care. Keeps up to date with evidence-based practice and research related to nursing and health care in the community. Ability to operate community equipment. Ability to identify and pass on urgent client information to the District Nurse / Team Leader.
  • Qualifications: 1st Level Nurse Registration (live registered with the NMC). Willingness to undertake further education around management of long-term conditions. Evidence of continuing professional development. Previous community nursing experience.
  • Skills: Planning / Organisational Skills. Ability to work across a seven-day week. Ability to undertake nursing skills and to prioritise visits in accordance with patient need. Ability to organise own workload without direct supervision. Ability to input data manually and electronically according to organisational policies. Work within agreed Policies and procedures.
  • Physical Skills: Ability to travel effectively and efficiently within the locality and at times, across NW Surrey. Dexterity to enable clinical skills to be performed, i.e., removal of sutures, venepuncture & IV therapy.
  • Communication: Team worker and ability to work alone. Ability to listen to and communicate with patients and carers in a compassionate and caring manner. Presentation skills or teaching to individuals e.g., patients and to small groups for example students.
