Croydon Health Services offers a truly integrated approach to paediatric nursing across our acute and community services. Supporting seamless pathways for children and young people between the hospital, their home and educational settings for children requiring short-term admissions to those with long-term and complex health needs. We are seeking an enthusiastic and experienced Community Children’s Nurse to join our dedicated team at Croydon Health Services NHS Trust for a 12-month maternity leave cover. This is an excellent opportunity for a motivated and compassionate nurse to work within a supportive and forward-thinking service, providing high-quality, family‑centred care to children and young people with a range of acute and complex health needs in their homes and community settings.

While previous experience in community children’s nursing is ideal, we also welcome applications from nurses with extensive Band 6 experience and transferable skills who are keen to develop their practice within the community setting. The successful candidate will have strong clinical and communication skills, be confident working both autonomously and as part of a multidisciplinary team, and be committed to delivering safe, effective, and compassionate care. You will be joining an experienced, friendly and passionate team who work collaboratively to support each other and deliver the best possible outcomes for children and families in Croydon.

Clinical Responsibilities:

  • Deliver high-quality, evidence-based nursing care to children and young people with acute, chronic, and complex health needs in the home, school, and community settings.
  • Undertake comprehensive nursing assessments, plan, implement and evaluate individualised care plans in partnership with children, families, and the wider multidisciplinary team.
  • Administer medications and carry out a range of clinical procedures (e.g. enteral feeding, tracheostomy care, wound care, and intravenous therapy) in accordance with Trust policies and clinical governance standards.
  • Monitor and respond to changes in a child’s condition, taking appropriate action and escalating concerns as required.
  • Support safe and effective discharges from hospital to home, ensuring continuity and coordination of care.

Communication and Coordination:

  • Act as a key point of contact for children, families, and other professionals involved in care delivery.
  • Establish and maintain effective communication with families, schools, GPs, secondary and tertiary specialist teams, statutory and voluntary services to ensure seamless, family-centred care.
  • Provide education, advice, and emotional support to parents and carers, empowering them to manage their child’s care confidently at home.

Leadership and Supervision:

  • Provide clinical leadership, supervision, and mentorship to junior nurses, students, and support staff within the team.
  • Contribute to induction, training, and development programmes for new and existing staff.
  • Support the implementation of service improvements and clinical governance initiatives in line with Trust priorities.

Safeguarding and Professional Standards:

  • Recognise and respond to safeguarding concerns, following Trust and national safeguarding policies and procedures.
  • Maintain accurate, timely, and confidential clinical records in line with professional and legal requirements.
  • Ensure practice is in accordance with the NMC Code of Professional Conduct and Trust policies.

Professional Development:

  • Take responsibility for maintaining own clinical competence and continuing professional development.
  • Participate in clinical supervision, reflective practice, and annual appraisal processes.
  • Engage actively in audit, research, and quality improvement activities to enhance service delivery.
