The postholder will support residential care home staff to positively manage and meet the health needs of individuals with a Learning Disability within the Croydon Area. The postholder will work collaboratively to ensure people living in care homes have access to enhanced primary care and to specialist services and maintain their independence as far as possible by reducing, delaying or preventing the need for additional health and social care services.

Responsibilities:

  • To identify and address health inequalities and ensure that long-term health conditions are identified and addressed.
  • To assess, advise and contribute to meeting the health needs of people with complex physical health.
  • Introduce a Training Analysis of residential care home staff to identify areas of further development and educative support.
  • Promote and support uptake of Annual Health Checks with GP Practices/PCN networks.
  • Review quality of Health Action Plans/Health Care Passports – Advise/support/facilitate identified health needs.
  • Support training and education.
  • Liaise with GP services to formulate and implement Urgent Care Planning in line with current guidance.
  • Advocate for residents.
  • To work with people who engage in behaviours that challenge (e.g. physical aggression to others, self-injurious behaviours etc.).
  • Support staff with MCA/BI decision making with GP services/involved health professionals and families.
  • Support care home hospital admission and complex discharge planning.
  • Ensure that care home staff feel supported and included as part of the MDT and integrated professional network.
  • Provide regular reports for LeDeR via the LAC and steering group.
  • To contribute to multi-disciplinary planning and meetings.
