Key Responsibilities:

  • Case-finding across all sectors, assessing and performing diagnostic evaluations.
  • Making recommendations for secondary prevention of osteoporotic fractures.
  • Working with other members of the falls services to ensure falls assessments are performed.
  • Providing input into the Falls Service as appropriate in line with agreed guidelines.
  • Co-ordinate and be responsible for the development, monitoring and maintaining of the Fracture Liaison Service in line with the overall Falls and Bone Health service.
  • Provide comprehensive and highly specialised assessment of falls and bone health in different clinical settings, including community based and hospital based clinics, GP practices, and patient homes.
  • Identify cases of patients in the community who require falls and bone health assessments, from various sources including fracture clinics, GP practices, and other community services.
  • Be an integral part of the falls team, working closely with the Consultant and different members of the Falls team.
  • Be actively involved in the assessment of treatment methods.
  • The post holder is an autonomous practitioner and clinical lead, providing specialist falls and bone health assessment, intervention and advice for Older People in Croydon who have fallen or are at risk of falling.
